What is liposuction?

Liposuction is the removal of unwanted fat from under your skin by breathing it in.

It can be performed on any area of the body that has excess fat, the most common being the stomach, thighs, buttocks and hips. These areas, especially in women, tend to store cellulite and fat, despite a good diet and regular exercise.

The different types of liposuction

LIPOSUCTION ASSISTED BY SUCTION, LIPOSUCTION OR SAL

This is the best-known liposuction procedure, which reshapes the figure by suctioning fatty deposits from certain parts of the body. Very fine cannulas are used to suck out the excess fat through very discreet incisions.

LASER LIPOSUCTION

The use of lasers has been a trend in the world of beauty and plastic surgery recently. This technique essentially uses the heat and energy of a laser to melt fat. In practice, if the fat is melted before it is removed, it is then removed in a gentler, less invasive way.

The laser option avoids the need for hospitalization and only requires local anesthesia.

VASER LIPOSCULPTURE OR ULTRASOUND ASSISTED LIPOSUCTION

Vaser liposculpture uses ultrasound energy to break down the fat, using a fairly large incision, and the ultrasound technology penetrates the body to make direct contact with the fat.

Surgeons say that this technique involves less swelling, bruising and blood loss, and is a gentler, less complex surgery. With this method, it is now possible to remove fat in areas that are known to be more difficult, such as the back and love handles.

However, the incision will need to be larger than a traditional liposuction incision to accommodate the ultrasound device, resulting in a larger scar.

POWER ASSISTED LIPOSUCTION

This technique uses a faster and more efficient machine to remove excess fat. It is the ideal choice if you have a large amount of fat to remove.

It works in the same way as traditional liposuction, except that the cannula is vibrating, which allows it to break down the fat more quickly.

HIGH DEFINITION LIPOSUCTION OR HDL

This is a much more extensive procedure that requires two steps. The goal of this surgery is to define and target the muscles in your body, primarily your torso and abdominal muscles, that are hidden beneath your fat.

This means that the fat must first be removed in the traditional way. The cannula is placed in very specific areas, where a significant loss of fat will promote the appearance of muscles.

Because it is a more stratified procedure, the operation and its recovery time is more important.

 

And after the procedure?

The treated area will be well wrapped with bandages to reduce swelling and minimize your pain. Results will not be visible immediately

It is important to buy your post-op lingerie beforehand, as it will help the healing process, limit swelling and effusions and promote fluid circulation: choosing quality lingerie is essential!

 

The swelling may last for several days or even weeks. It will be important to visit your surgeon after the operation to check that the post-operative process is going well.

Recovery after liposuction

Post-operative recovery from liposuction takes a few weeks. Your surgeon will have done everything possible to ensure that the incision is minimal and that swelling is kept under control.

> You have to be over 18 to have breast surgery 

P.B: False, especially in the case of patients with malformations, asymmetries that often reveal themselves quite early in adolescence. It can be useful to operate on the patient so that her development is in the right direction! Of course, in this situation, parental permission is required from both parents. In the case of cosmetic surgery, the question of waiting until the age of majority is inevitably posed, in order to finally reach a majority on paper and also a certain psychological maturity.

 

> Healing is long and painful

P.B.: False. Healing is a natural physiological process that lasts on average between 12 and 18 months. On the painful side, normal wound healing is not painful. There may be an itchy, scratchy feeling around the scar which is completely natural. This is simply the inflammatory process which generally takes place between 3 and 6 months after the operation. On the other hand, sometimes these scars can actually be painful, in the context of a pathological context that is outside the natural process.


> It is no longer possible to breastfeed after a breast augmentation

P.B.: False. After a breast augmentation it is perfectly possible to breastfeed.

However, there are different approaches, different areas through which the breast can be operated. The approach at the areola is an approach where we will be more easily exposed to the milk ducts that lead the milk from the mammary gland to the nipple. In certain situations these milk ducts can be damaged and therefore responsible for a difficulty in breastfeeding in the context of the placement of breast implants.

In the case of submammary or axillary approaches at a distance from these ducts, the possibility of breastfeeding is of course maintained. In the case of breast reduction surgery, which will reduce the breast and in particular the gland, in this situation the ducts are often damaged. The possibility of breastfeeding is therefore greatly reduced or even impossible!


> The operation can leave different types of scars

P.B: True. Most of the time the scar is normal, i.e. fine, it is red for the first few months and eventually turns white. In certain cases, particularly on white, very white or black skin, we can encounter pathological scars of two kinds: a hypertrophic scar. It is a scar that will be thick, red, painful and evolving, which ends up involuting a little, which can be responsible for pain and a real aesthetic discomfort and of course all types of scarring is evoked in the pre-operative consultation.


> You have to change your prostheses after a certain time

P.B: True. Breast implants are implantable medical devices and therefore foreign bodies. They may have the best qualities close to the texture of a breast, but when they are inserted at a given moment, the patient’s breast will change.

With weight gain, weight loss, shocks in the intimate life, during sports and therefore the prostheses will be subjected to mechanical constraints. At present, prostheses that have a life span of 10 years will not be systematically changed, but the patient will be offered an imaging scan every two years to monitor the state of the prosthesis, to check for a shell, i.e. a rigid capsule around the implant. Of course, there may be cases where the prostheses will have to be changed before 10 years. For all these reasons, the lifespan can be variable and therefore in these cases the change can be earlier.

Apart from all that, beyond 10 years, we can estimate that a prosthesis can sometimes have a lifespan of 15 or even 20 years. We will not wait until there is a major problem such as a broken prosthesis. It is always easier to change implants that have few problems than implants that are completely ruptured.


> Post-operative lingerie is part of the healing process

P.B: True. Somehow the support of a comfortable and technically supportive lingerie allows to decrease the shearing force on the scars, allows to decrease the weight of the implant on the scar and thus to participate in the healing process being relatively soft and going on in a correct way, without mechanical constraints.


> In France there is a lot of emphasis on lingerie

P.B: True, this may not have been the case immediately, but with the lingerie that has developed in the technical field of different types of fabric, and particularly at Embody, beyond the technical aspect of refinement, this importance has taken on greater importance. > Lipofilling gives the same result as breast implants

P.B: I would say true and false! The aim of lipofilling, like breast augmentation by prostheses, is to increase the volume of the breast. On the other hand, the wish of some patients is to be able to live in the neckline, which is sometimes called the social breast; the prosthesis will be able to provide this thanks to its power to push forward, which lipofilling could provide a little less.

It is possible to have a very natural result with prostheses as with lipofilling. The difference is that there is a foreign body with a predefined prosthetic volume, which is not necessarily possible with Lipomodelling.


> The surgeon can refuse a patient’s request

P.B: True, there may be requests that are unfounded. Requests from a fragile patient or within the framework of certain dysmorphophobia where the image of the patient’s body is not at all the reality, and therefore this is something that we must try to identify during the consultation in order to be able to detect all patients who could be a bad candidate for breast surgery. A patient who might be dissatisfied or simply not accept the surgery and experience things even worse afterwards. Hence the importance of knowing how to say no if enough elements are present to lead to a failed surgery.

How to choose a bra after breast surgery?

Why do you need to wear a post-surgery bra after breast surgery?

All about breast lipofilling!

Breast augmentation by lipofilling is a surgical procedure to remove fat cells from the patient and inject them into the breast to increase volume, correct asymmetry or perfect the result of breast augmentation with prostheses. It can also be prescribed for women who have had breast cancer. It is important to know that a breast is essentially made up of a gland and fat. Breast lipofilling is a technique of cosmetic surgery of the breasts which allows to obtain a durable and natural result with almost invisible scars.

Very often breast lipofilling is associated with liposculpture, which allows to take advantage of a general anaesthesia to reshape the figure and thus recover the fat necessary for the operation.

This technique makes it possible to gain a bra cup. It is possible to perform several operations at intervals of at least 3 to 6 months to obtain a larger breast. In addition to this, the patient benefits from liposuction of areas of excess fat such as the thighs, stomach or buttocks…

Unlike breast implants, this method gives more natural results without scars.

Advantages of this technique

Breast augmentation using the lipofilling technique has three advantages:

Lipofilling as part of breast reconstruction

Lipofilling is recommended for breast reconstruction following a total mastectomy. An operation like this reconstructs the volume of the breast and improves the quality of the tissue.

As a recurrence is always possible, the surgeon must always carry out a pre-operative clinical examination (examination of the cellular tissue, ultrasound of the chest wall, sometimes biopsy).

In the case of partial breast reconstruction after lumpectomy and given the risk of recurrence, reconstruction by breast lipofilling can only be carried out at least 3 years after clinical and radiological monitoring.

How does it work?

 

  • The collection of fat cells

Your surgeon will start the procedure by determining the locations where the fat will be collected and the sites for reinjection.

Then a sample of fat tissue is taken through a micro-incision with a fine suction cannula or during a conventional liposuction. The fat cells are extracted from a discrete area of the body where there is a reserve or even an excess of fat cells (usually in the abdomen, hips, knees, inner thighs, buttocks).

The fat cells collected are then subjected to sterile centrifugation for several minutes in order to separate the cells that will be injected (intact fat cells) from those that should not be injected (fat cells destroyed by the collection, non-fat cells such as blood cells).

 

  • Reinjection of fat cells into the breast

This is carried out using micro-cannulas allowing incisions of around 1-2 mm.

The duration of breast lipofilling can vary depending on the quantity of fat to be injected and the number of areas concerned. The duration of the procedure is generally between 30 minutes and two hours.

 

  • Follow-up after breast lipofilling

The operation is normally carried out with a day hospitalization.

Post-operative pain is mild in most cases.

It is normal that an oedema appears in the 48 hours following the operation. It will disappear completely within 5 to 15 days after the operation.

Bruising may occur in the fat collection and reinjection areas but this will disappear within 5 to 21 days. The procedure is less extensive than a breast augmentation with implants. It is possible to resume normal activity 2 to 4 days after the procedure.

 

  • How are the scars after lipofilling?

Compared to breast augmentation with implants, breast lipofilling will cause little risk of visible scars. Why is this? Because the cannula used to remove and transplant the fat cells is extremely fine.

The result is very natural since the reinjected fat corresponds to the normal tissues of the body and the breast.

However, smoking increases the risk of surgical complications in any operation. Quitting smoking 6 to 8 weeks before the operation eliminates this additional risk. If you smoke, don’t forget to tell your doctor, surgeon and anaesthetist.

Buttock lipofilling: the alternative to implants

Buttock augmentation is a very popular cosmetic surgery procedure. But why?

The buttocks are an essential part of the body. They constitute the harmony of the silhouette. No two buttocks are alike, they can be flat, round, bulging, large or small, and in the course of a lifetime they can change their appearance. How does it work?

First of all, you will have an interview with your surgeon to explain your project. He will ask you various questions about your motivations and your medical history, in order to rule out any contraindications.

A clinical examination will be carried out in order to know how much fat will be removed and in which area. He will evaluate the morphology of the buttocks, the silhouette as well as the amount of skin and the flexibility of the tissues.

Buttock lipofilling is a plastic surgery procedure performed under general anaesthesia and lasts about 2 to 3 hours. It is performed on an outpatient basis and may require a 24-hour hospital stay.

First of all, your surgeon will perform liposuction, he will remove fat from the areas that you have previously targeted with him. This is done with a very fine cannula and, depending on the quantity of fat to be removed, one or more micro incisions may be necessary, leaving invisible scars.

The fat will then be purified by centrifugation, decantation or simple washing. The aim of these manoeuvres is to separate the good quality fat cells from the impurities (blood, serum, cellular debris).

Then this purified fat will be reinjected into the buttocks, at the level of the gluteus maximus muscle: this muscle has many blood vessels, which favours the chances of the graft taking. To do this, the surgeon makes very small incisions into which fine cannulas are inserted to reinject the fat into the chosen areas.

This process is carried out gently, with a quantity of fat adapted to each case, in the appropriate areas to give the best possible morphology, both in terms of volume and contours.

At the end of the procedure, a support panty is put on for optimal maintenance of the grafted area.

 

Are there any risks?

Even if this procedure is 100% natural and biocompatible, as with any surgical procedure, there are associated risks.

For buttock lipofilling, you should pay attention to certain points:

  • Blood clots, pulmonary embolisms: blood clots in the legs can travel to the heart or lungs and cause serious complications. Patients should be assessed for any pre-existing risk of blood clots.
  • Infections: Surgical infections are always a risk, especially with large volume fat transfer. It is important to use a meticulous sterile technique, and all patients receive appropriate antibiotic prophylaxis.
  • Changes in sensation: any area operated on will swell and the swelling may cause nerves to stop or malfunction. Sometimes the nerves have to be cut during the operation. You may experience numbness, which is usually temporary, but in some cases it may be permanent. In this case, there will be no way to regain sensation if the nerve function does not return.
  • Poor healing: during the operation the wounds are well closed, but if your stitches pop out, the scar could widen or look bad. Fortunately, the incisions for this procedure are very small, but there is always a risk of scarring.
  • Contour irregularities: When fat is moved from one area to another, there may be some asymmetry. Your surgeon will give you very specific instructions on activity levels, sleeping positions and compression garments. You will need to follow these instructions to get the best results!
  • Fat embolism: It is possible that the injected fat can enter the bloodstream and reach other organs (lungs, brain, etc.). This risk is higher if the fat is injected into a muscle, if it is injected at abnormally high pressures or if very small cannulas are used. Talk to your surgeon about the technique he or she uses.

 

And after the buttock lipofilling?

Generally, the aftermath of this operation is not very serious. Classic painkillers can help you to bear the slight pain you may feel. Bruises and swelling may appear in the donor areas, but will disappear fairly quickly. You will be advised to sleep on your back and to sit on the lower part of your buttocks for 3 weeks. A week’s leave from work will probably be necessary if your activity can damage your scarring. Sports activities can only be resumed after 6 weeks.

When are the results definitive?

The final result will be visible between 3 and 6 months after the operation. In the first few months you will see a loss of fat that varies between 30 and 50%. However, it is recommended that you keep your weight stable. The results are permanent, but as it is fat, as for all parts of the body, if you lose weight you will also lose volume and vice versa!

Reparative surgery after pregnancy : the Mommy makeover!

Following one or more pregnancies some parts of the silhouette may have undergone modifications such as breasts or abdomen. The “mommy makeover” is a plastic surgery that aims to redraw the figure, in a single operation.

 

What exactly is the mommy makeover?

The ‘mommy makeover’ is a surgical procedure that aims to touch up parts of the body that have been damaged, so that they regain their original appearance, before the pregnancy or pregnancies, in a single operation. Depending on the needs, these different operations can be carried out:

– breast surgery: breast augmentation, breast lift

– abdominal belt surgery: liposuction, abdominoplasty

– gluteal volume restoration surgery

– intimate surgery to rejuvenate loose lips or vagina distended by pregnancies

 

An increasingly common request

Consultations for the mommy makeover are increasingly in demand. Women often want to regain their pre-pregnancy breasts or a flatter stomach, without stretch marks or excess skin. Abdominal muscle relaxation, due to excessive stretching of the abdominal muscles, is also a frequent reason for consultation.

Conditions :

– It is necessary to wait at least 4 months after delivery or end of breastfeeding, if the mother is breastfeeding

This is a procedure that can be potentially dangerous if safety and surgical common sense are not followed:

– Operation time should not exceed 4-5 hours

– Risk of bleeding must be properly assessed by surgeon

– Good health of the patient

– Overweight is a contraindication because it increases the risk of phlebitis, bleeding and infections

– It is important not to suffer from psychological disorders including dysmorphophobia

– No pathology that could put the patient at risk: diabetes, kidney failure, heart or respiratory failure, history of pulmonary embolism or phlebitis


– Normal blood tests

– Normal clotting balances

– Patient expectations should be reasonable

 

An intervention practised, but little known in France

In France, we are in a safe medical logic where we prefer to avoid associating too many acts during the same intervention. It is therefore necessary to carefully select patients who are healthy with a stable weight. It is also important to ensure that the surgical gestures will not be too long, otherwise the interventions will have to be separated into several stages, spaced 3 months apart on average.

→ The most important point is to ensure that the preoperative haematology assessment (which also includes the analysis of the patient’s clotting abilities), is perfectly normal.

Anaesthesia, price and healing

This procedure is performed under general anaesthesia because it is long and multi-zone, not allowing to perform extensive local anaesthesia.

After a mommy makeover it takes 14 days of convalescence minimum, but this may vary depending on the number of operations performed.

In terms of price, everything will again depend on the number of related acts but it can range from €6,000 to €20,000.
